To be certified as a Small Business Enterprise with the School District of Palm Beach County this completed application and a notarized affidavit must be submitted to te School District Office of Diversity. You are required to be recertified every three (3) years from your initial certification date. DIRECTIONS: Check "Certification" if you are applying for the first time. Check "Recertification" if you are applying for the three-year SBE recertification. Enter your name and click OK. Follow the directions provided on the application. On the last page of the application there is a link to the affidavit. Print the affidavit, complete, sign and mail the notarized affidavit. Your application will not be processed until the notarized affidavit and the non-refundable $150.00 processing fee has been received. For further information call (561) 434-8508.
